This notice explains what happens to information sent through the Lam Pharmacy new patient form.
Information we collect
We collect only the details shown on the form: your name and phone number, your call or text preference, your doctor or practice contact details, insurance-card images or PDF files, your language selection, and your consent.
Why we use it
Lam Pharmacy uses this information to contact you, begin pharmacy intake, and check insurance information or coverage. Submitting the form does not guarantee coverage, pricing, or that a prescription can be filled.
How it is handled
Submissions are encrypted and stored in private access-controlled storage. Access is intended only for authorized Lam Pharmacy staff. Internet transmission and storage carry risk, so please submit only the requested information. Do not add medication lists, medical history, Social Security numbers, or payment-card information.
Retention and deletion
Lam Pharmacy keeps a submission only as long as reasonably needed for intake, insurance verification, operational, or legal purposes. Contact the pharmacy if you want to ask about access, correction, or deletion; some records may need to be retained when required by law.

This website does not claim that the intake form or the site is HIPAA compliant. This form is not monitored for emergencies and does not provide medical advice.
